The immune response is both a signal and a target.
The clinical problem
PJI can become a disease of both bacterial persistence and host immune failure.
Implants, biofilm, and disrupted bone create a protected niche in which bacteria are difficult to eradicate. At the same time, prolonged antigen exposure can erode protective immunity—helping explain why surgery and antibiotics alone may not restore control.
A preoperative serum biomarker classified one-year outcomes in a 52-patient pilot cohort drawn from 18 centers.
Immune checkpoint blockade reduced infection burden and adverse bone reaction in a humanized mouse study.
